Chiropractic Neurology Evaluations

Our evaluations are thorough, lasting at least 60 minutes. To ensure accuracy, please complete all forms fully, reflecting the patient's authentic self. You do not need to score the forms.

Steps to Prepare:

1. Complete the Necessary Forms:

   • Child History Form or Adult History Form

   • Cognitive Checklist

   • Sensory Function Checklist

   • Adult Primitive Reflex Checklist

2. Submit Your Forms:

   • These are fillable PDF forms. Save them to your computer and email the completed forms to our clinic at admin(at)enlivenwv.com or fax them to (304) 278-3348.

3. Prepare for Your Appointment:

   • Ensure a good night's rest and have a healthy breakfast beforehand.

   • Be aware that you or your child may feel tired and possibly emotional after the examination. This is normal as we will be activating and testing underperforming areas of the brain.

What to Expect:

At the end of the examination, we will provide neurologic exercises and a personalized treatment plan.
